Why Clinic Scheduling Is the Most Complex in Healthcare

A clinic is an ecosystem of interconnected departments. Each department has its own scheduling constraints, but all must work together. Departments with Different Logics

  • A&E: 24/7, staffing varies by hour (more in evenings and weekends), total unpredictability of workload.

Medical On-Call

On-call physicians ensure medical continuity outside normal hours. Surgeon on-call, anaesthetist on-call, internist on-call: each specialty has its on-call rota. These rotas are often monthly and must be integrated into the global schedule. A surgeon on-call over the weekend cannot be scheduled in the operating theatre on Monday morning without adequate rest.

Nursing Rotations

Nurses rotate between morning, afternoon and night shifts. Rotations must be fair (not always nights for the same people) and compliant with rest rules (11 hours between shifts, weekly rest, maximum consecutive nights). In a 20-nurse department, managing these rotations manually is a logistical nightmare.

Minimum Staffing per Department

Each department has staffing standards. The surgical inpatient unit needs X nurses per occupied bed. A&E needs at least Y nurses and Z physicians at night. The operating theatre requires a complete team per active operating room. These minimums are non-negotiable.

Import or create the on-call rota for physicians by specialty. On-call duties appear in the global schedule with their impact on rest and subsequent shifts. A physician on-call on Sunday cannot be scheduled in outpatient on Monday morning if rest periods aren't met.

Operating Theatre Planning

Create operating programmes by room and day. Each procedure requires a team: surgeon, anaesthetist, scrub nurse, circulating nurse. Shyfter verifies the team is complete for each scheduled procedure. Cleaning time between operations is integrated.

Replacement Management

When a staff member reports an absence, Shyfter identifies available and qualified replacements in the relevant department. If no internal replacement is available, you can call on other departments or an external pool. Night Work and Rotations

Night work in a clinic is structural. Rules are strict: maximum consecutive nights, compensatory rest, enhanced medical surveillance for regular night workers. A schedule forcing 5 consecutive nights on a nurse is non-compliant. Shyfter checks these limits automatically.
